Thank you for your interest in organising an assessment with Dr Prakash Grover.
Please complete one of the following referral forms;
For a person under the age of 18, please complete the Referral Form.
If the person is over the age of 18, please complete the Adult Referral Form.
If you are an RTLB/SENCO/SCHOOL, please complete the RTLB Referral Form.
Once you have completed the appropriate referral form, please submit.
When our office receives the referral form, we will contact you to arrange 2 appointment times.
Each session can be from 45 minutes to 1.5 hours depending on the circumstances. We will always
book the maximum time of 1.5 hours.
The cost of the assessment will be discussed with you when we contact you to make the
appointments.
A confirmation email will be sent once this process has been completed.
